collectively referred to as “Integra Group”, “we” or “us”) respect and are committed to the protection of your personal information and data privacy. This notice shall clarify data collection, process, and storage with the purview of regulated statutory body which included but not limited to
  1. Personal Data Protection Act 2010
  2. Malaysian Communications and Multimedia Commission Act (1998).
By using this website and/or providing your personal information to us, you are consenting to this Privacy Notice. This notice shall be reviewed and amended from time to time. The latest updated version shall be applied and supersede all previous versions which include leaflet, or any hardcopy versions found within incorporation premises.

B. Personal Information 

We may collect a variety of your Personal Data including but not limited to your name, address, gender, marital status, contact details, email address, nationality, race and religious beliefs, education background, occupation, information about your organization and your designation within the organization, salary range, information in audio/or video format (including voice, video recording, closed-circuit television (“CCTV”) or security recording, images (including photographs), biometric information and location tracking/GPS information, medical information (such as blood type, finger prints, DNA and laboratory reports), insurance information, bank account information, credit profile and other information deemed necessary. 

C. The source of personal information. 

a. Patient or customer We shall collect your personal information directly or indirectly from your legal representatives (family members, guardian, next of kin), agents such as medical tourism agents, your employer, private guardian, insurer, referral physician, private clinics, Social Security Organization (PERKESO) send us completed enquiry, application and/or registration forms via various means including online and physical hardcopies at public venues or in our premises. Your personal information may also be collected via cookies via the use of our website.

b. Independent consultants or potential independent consultants, We shall collect your personal information directly from you or indirectly from headhunters when you and/or our headhunter send us completed enquiry and/ir application forms or curriculum vitaes via various mean, including online or physical hardcopies. 

c. Vendor or supplier or service provider We shall collect your personal information directly or indirectly from your employer or credit reference agencies when tendering for projects when send us completed enquiry and/order application forms via various means, including online and physical hardcopies. 

D. Obligatory of Personal Information

All information requested for the relevant forms via hard copy or online are obligatory to be provided by you unless stated otherwise. Should you fail to provide deems necessary information, we may be unable to render you with the relevant services or continue our dealings with you.

E. Intention of collection and processing personal information.

Personal information is collected and process by us as required or permitted by law and to give effect to your requested commercial transaction, including the following:

For patients or potential patients/customers

  • to process your requested medical/healthcare services.
  • to facilitate your participation in any contests, events or membership programs.
  • to administer and communicate with you in relation to our services and/or events.
  • to monitor utilization of membership benefits.
  • for purposes related to insurance, appointed third party administrators and/or managed care organizations appointed by your insurers or employers.
  • to respond to inquiries from you and/or authorize next of kin.
  • to conduct internal statistical analysis and analysis of patient case studies.
  • to contact you regarding our products, services, upcoming events, promotions, advertising, marketing and commercial materials which we may feel interest you.

For independent consultants, potential independent consultants

  • to facilitate your medical practice within our group, including sharing your personal data with other independent consultants within our subsidiary for purposes of peer review.
  • to respond to inquiries from you and/or authorize next of kin.
  • to administer and communicate with you in relation to your medical practice and
  • to conduct internal statistical analysis and analysis of patient case studies.

For vendors, suppliers or service providers

  • to commence a business relationship with us (for example, as a vendor, service provider or a business partner)
  • to process your credit account application.
  • to assess your credit worthiness.
  • to administer and give effect to your commercial transaction (tender award, contract for service, consignment agreement); and
  • to process any payments relevant to you.

For all individuals

  • to operate our premises in a manner which is physically safe, secure, health fit and safety requirements.
  • for internal investigations, audit, or security purposes.
  • to comply with our group’s legal and regulatory obligations in the conduct of its business and our group’s internal record management.
  • to ensure that the content from our website is presented in the most effective manner for your and for your computer and/or device through passive tracking via cookies and other similar tools.

F. Assurance of Data Disclosure and Transfer of personal Information (within or outside of Malaysia)

Within Integra Groups of Companies.

Your personal information provided to us may be processed by entities (within or outside of Malaysia) and Integra Group (including related companies, subsidiaries, holding companies and associated companies), especially when you are an employee of any company within Integra Group. 

Integra Group will ensure that: 

a. access to your personal information is restricted to staff who are contractually required to process your personal information in accordance with their respective job requirements and 

b. only deems necessary information is release to relevant employees. 

Third Parties Classes 

Your personal information may be disclosed or transferred to relevant third parties (within or outside of Malaysia) as required under the law, pursuant to relevant contractual relationship (such as appointment of third-party service providers) or for the purposes stated in section E. 

In the event of a potential, proposed or actual sale of business, disposal, acquisition, merger, or re-organization (“Transaction”), your personal information may be required to be disclosed or transferred to a third party because of the Transaction. You hereby acknowledge that such disclosure and transfer may occur and permit Integra group to release your personal information (only as required) to the other party and its advisers/representatives. 

Personal information may be disclosed to the third parties’ classes with circumstance of 

  1. The ministry of health (MOH) or any other statutory or non-statutory authorities or bodies with authorized jurisdiction.
  2. Relevant accreditation bodies such as the Malaysian Society for Quality in Health (MSQH), Joint Commissioner International (JCI).
  3. Our independent consultants and specialists within the group of companies.
  4. Your insurers, the appointed third parties administrators and managed care organization by insurers or employers.
  5. Third parties appointed by us to provide services to us or on our behalf (such as auditors, lawyers, company secretary, printing companies, consignment vendors, medical devices maintenance providers, information system vendors, contractors, conference/event organizers and other advisers)
  6. The respective foreign embassies of foreign patients who received treatment in our Group of companies; and
  7. Law enforcement agencies, including the local police.

Links to other sites 

Content in Groups of companies’ websites may include links to third party websites. Our Group is not responsible for the content on such linked third-party websites and users should observe the separate privacy policies applicable to the respective third party’s website. 

Cookies 

Integra Group and its subsidiary websites may use cookies and similar tools across our websites to improve their performance and enhance your user experience. Cookies are small text files which a website may put on your devices when you first visit a site or page. The cookie will help the website, or another website, to recognize your device the next time visitation. Cookies can function to remember your username and preferences or to analyze the performance of website. 

Certain cookies contain personal data for example if you click “remember me” when logging in, a cookie will store your username and other data you provided. Most cookies will not collect information that identifies you and will instead collect more general information such as how users arrive at and use our websites, or users general location. 

Where personal data is collected such as personal data will only be used for its intended purpose only. Such as to respond to your message or deliver the requested services. 

H. Right to access and correct personal information. 

You have the right to access and correct your personal information held by us to certain subject and exemptions. We will make every endeavor to ensure your personal information is accurate and up to date therefore you were requested to notify us directly for such changes.

For each data access request, prescribed fees may be levied, and we may reject such request should the request fall under any exception in the PDPA or any other applicable law. 

I. Private Healthcare Facilities and Services Act 1998 

Please note that the manner in which our groups of company process your personal data will be subject to other industry-specific laws and guidelines including the Private Healthcare Facilities and Services Act 1998 (“PHFSA”) and Ministry of Health guidelines. For example, access to medical records will be in accordance with the PHFSA. 

J. Limiting the process of personal information, further enquiries, and complaints. 

You may limit our use of your personal information, on marketing communications from us or withdrawal consent to receive such material, or you have any queries and concerns about the Privacy Notice or complaints in respect of your personal information. You may contact or write to us below point of contact. We will do so within the time-period required by applicable data protection related law.

K. Conflict 

In the event of any conflict between this English language Personal Data Protection Notice and its corresponding Bahasa Malaysia Personal Data Protection Notice, the terms in this English language Notice shall prevail.
